MyChart Terms and Conditions

Use of MyChart requires that you agree to the following terms and conditions. MyChart allows you to manage appointments, get lab results, contact providers, access medical records, manage medications and pay bills. If you agree, please click "ACCEPT" at the end of this document.

"Cigna" is a registered service mark of Cigna Intellectual Property, Inc., licensed for use by Cigna Corporation and its operating subsidiaries. Cigna Corporation is a holding company and is not an insurance or an operating company. Therefore, MyChart is provided exclusively by such operating subsidiaries and not by Cigna Corporation. "Cigna" may refer to Cigna Corporation itself or one or more of its subsidiaries, but when used in connection with the provision of a product or service, always refers to a subsidiary.

Urgent Issues
You should not use MyChart for assistance with urgent or critical issues. In case of emergency, you should call 911 or your provider or go to an emergency room.

Health Information
Cigna provides health information through MyChart for informational purposes and to promote consumer health. This health information is not a substitute for proper professional medical evaluation, advice, and care. Always consult with your doctor for appropriate examinations, treatment, testing, and care recommendations. Do not rely on this health information as a tool for self-diagnosis or an alternative to consultation with your provider.

E-mail Communication
You will receive an email notification when MyChart has new information available to you. Anyone with access to your e-mail account may see the notification. Although these notifications do not contain your personal health information, you may prefer to keep the notification private. Please consider this when providing an e-mail address.

IDs and Passwords
Never share your MyChart user ID or password. You are responsible for all activity performed with your user ID and password on MyChart. Access shall not be granted to any entity that runs, executes or writes automated (bot) scripts. Access should only be assigned to human beings.

Termination of Access
Cigna reserves the right to suspend or terminate your access to MyChart. Your use of MyChart is conditioned upon your compliance with the following:

1. Do not transmit any unlawful, threatening, abusive, libelous, defamatory, racist, obscene, vulgar, pornographic, profane or indecent information. This includes, without limitation, any transmissions that are or that encourage illegal conduct. This ban includes anything that would constitute a criminal offense, give rise to civil liability or otherwise violate any local, state, national or international law.
2. Never transmit comments containing harassing or offensive language, using sexual references, sexual nicknames, racial slurs or rude or deliberately offensive comments.
3. Do not engage in disruptive activity online, including excessive use of scripts, sound waves or scrolling (repeating the same message over and over).
4. Do not transmit any information, software or other material that violates or infringes the rights of others.
5. Never transmit any information, software or other material that contains a virus, worm, Trojan horse or other code that has contaminating or destructive properties.
6. Do not transmit any information, software or other material for commercial purposes.
7. Never pretend to be any other person or entity. Site users also may not falsely represent their professional or other affiliation with any person or entity.

You agree that Cigna determines whether any information transmitted or received violates these terms and conditions.

You also agree to indemnify and hold Cigna and our employees, officers, directors and agents harmless from any claim or demand, including reasonable attorneys' fees, made by a third party arising from any information or material you contribute to MyChart.

Cigna has no obligation to monitor MyChart. But we reserve the right to monitor any and all information transmitted or received through MyChart. Cigna may examine, record, copy and use any information on MyChart in accordance with Cigna's Privacy Policies. We reserve the right to remove any information or materials, in whole or in part, from MyChart.

When using MyChart, do not disclose anything that may compromise your personal safety. You do not need to continue any conversation online that makes you feel uncomfortable. Protect your safety and privacy.

Eligibility for MyChart
You must be 18 years old or older to obtain a personal MyChart account. If you are a minor child's parent or guardian, you may request access to the minor child's account by following the instructions in the paragraph below.

Proxy Access
You may request proxy access to the MyChart account of an individual under the age of 12 by completing the MyChart Request for Proxy form available at the patient's primary clinic and submitting it to the clinic. You will only be granted access if you have the parental rights or legal guardianship over the MyChart account holder and only to the extent you can show you have the legal right to the account holder's medical information. If granted, you will be provided access to a MyChart account for both you as the proxy and the account holder. If your legal relationship with the account holder changes, you must inform the patient's primary clinic immediately. Cigna reserves the right to revoke proxy access at any time for any reason.

Cigna Content and Others' Content
While using MyChart, you may access non-Cigna content or you may view non-Cigna website content by following links to non-Cigna websites or content. Categories that include non-Cigna content or links may include a highlighted statement informing you of the non-Cigna content. We do not review, control or endorse the content of non-Cigna categories, or any non-Cigna website, and therefore will not be responsible for their content or accuracy. Your access to non-Cigna content and non-Cigna websites is at your sole risk.

General Disclaimer and Limitation of Liability
All information on MyChart is provided "AS IS" and without warranties of any kind, either expressed or implied. This includes, but isn't limited to, the implied warranties of merchantability, fitness for a particular purpose, or non-infringement. All information is presented without any warranty as to its reliability, accuracy, timeliness, usefulness or completeness. Cigna doesn't guarantee that access to MyChart will be uninterrupted or error-free. You use MyChart at your own risk. We assume no responsibility for any circumstances arising out of the use, misuse, interpretation or application of any information supplied on MyChart.

By accessing MyChart, you agree that under no circumstances or any theories of liability under international or civil, common or statutory law (including but not limited to strict liability, negligence or other tort theories or contract, patent or copyright laws), will Cigna be liable for damages. We're not liable for damages of any kind occurring from the use of MyChart or any information, goods or services obtained through MyChart. This includes direct, indirect, consequential, incidental or punitive damages (even if Cigna has been advised of the possibility of such damages), to the fullest extent permitted by law. Some jurisdictions don't allow the exclusion or limitation of certain damages, so some of these limitations may not apply to you.

Web-Enabled Mobile or Handheld Devices
Certain web and smartphone applications of MyChart have been designed to run on a personal computer, standard laptop or web-enabled mobile device. If you view or use MyChart on a mobile or handheld device you may experience some display and functionality limitations. You may experience varying wireless network coverage and speed, depending on your geographic location and telecommunications provider. MyChart should not be used while operating any motor vehicle, or in any other manner that might endanger yourself or others. If your mobile device uses global positioning system ("GPS") technology, trackers, or other location tracking tools, your location information may be used for purposes of determining your location when using a function on MyChart that uses such information. While using MyChart, if you allow your location information to be obtained, we will use this information solely to return your estimated location and to provide you with more personalized results and/or services. Cigna does not monitor your GPS tracking or location information, and does not save this information. We will not use this information to identify you or disclose this information to any third parties.

By using MyChart you expressly agree and acknowledge that Cigna shall not be responsible for any of the following: (a) performance limitations and/or failures experienced in connection with any wireless or Wi-Fi service used to access MyChart; (b) data transmitted through wireless access that may be intercepted by unauthorized persons; or (c) your failure to physically secure and/or monitor your personal computer, laptop or wireless device or to protect your passwords, resulting in unauthorized access to MyChart and/or your Cigna account or plan information.

While using any web-enabled mobile or handheld device to use MyChart, standard mobile phone carrier and data usage charges will apply. You expressly agree and acknowledge that the use of any mobile or handheld device is at your sole risk and shall be in accordance with all applicable local, state and federal laws.

Updates and Reverse Engineering
By using MyChart, you agree to receive any applicable software updates that may be issued from time to time by Cigna. You agree to accept and install such updates as part of your use of MyChart. You also agree not to reverse engineer, modify, decompile, adapt, translate, disassemble, or otherwise attempt to discover the source code of MyChart and/or any Cigna application.

Viruses, Worms, Use of Files Cigna cannot and does not guarantee or warrant that files available for downloading from MyChart are trouble free. We cannot guarantee there are no viruses, worms, Trojan horses or other code that has contaminating or destructive properties. Cigna doesn't make any express or implied warranties, representations or endorsements (including respect to the files available for downloading from MyChart). In no event will Cigna be liable to you or to anyone else for any decision made or action taken by you or anyone else in reliance on results obtained from use of files downloaded from MyChart. You may download and print these files only for personal use. Permission to reprint or electronically reproduce any document or graphic in whole or in part for any reason except for personal use is expressly prohibited unless prior written consent is obtained from the appropriate copyright holder.

Privacy
Cigna's Privacy Policies and the privacy policies and practices of non-Cigna entities may be different. We suggest you review the privacy policies related to any non-Cigna content that you may visit from MyChart.

The Digital Millennium Copyright Act
Cigna reserves the right, but not the obligation, to end your access to MyChart if we determine in our sole and absolute discretion, that you are involved in infringing activity. Infringing activity includes alleged acts of first-time or repeat infringement, regardless of whether the material or activity is ultimately determined to be in violation of this agreement or any law or legal requirement. Cigna and our service providers accommodate and do not interfere with standard technical measures used by copyright owners to protect their materials. Cigna has implemented procedures for receiving written notification of claimed infringements and for processing such claims in accordance with the Digital Millennium Copyright Act. Please send any written notice regarding any infringement of copyright or of other proprietary rights to our designated agent. You'll need to include:
A. A physical signature of a person authorized to act on behalf of (i) the owner of an exclusive right that is allegedly infringed or (ii) the person defamed.
B. Identification of the copyrighted work you claim to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single notification, a representative list of such works at that site.
C. Identification of the material that is claimed to be infringing, or to be the subject of infringing activity, including information reasonably sufficient to permit us to locate the material.
D. Information reasonably sufficient to permit us to contact you-such as your address, telephone number and/or electronic mail address.
E. A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright or other proprietary right owner, its agent, or the law.

If you choose to access MyChart from outside the United States, you do so on your own initiative. You're responsible for your compliance with U.S. and local laws, if and to the extent that local laws are applicable.

Software is subject to United States export controls. You cannot download, export or re-export any software into any country (or to a national resident of a country) to which the U.S. has embargoed goods. This restriction includes Cuba, Iraq, Libya, North Korea, Iran and Syria. It's also illegal to download, export or re-export software to anyone on the U.S. Treasury Department list of Specially Designated Nationals or the U.S. Commerce Department's Table of Deny Orders. You represent and warrant that you are not located in, not under the control of, and are not a national resident of any such country or on any such list. You agree to comply with U.S. export control laws. And you agree to not transfer any software or other content from MyChart to a foreign national or foreign country in violation of those laws.

Trademarks
Please note the references throughout MyChart to various trademarks or service marks. Whether registered or not, these marks mean the designated product or service is the property of its respective owner.

Changes and Applicable Law
Cigna reserves the right to make changes to MyChart and these terms and conditions. We encourage you to review MyChart and these terms periodically for updates or changes. Your continued access or use of MyChart shall be deemed your acceptance of this these terms and conditions and any changes and the reasonableness of these standards for notice of changes. Use of MyChart will be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania without giving effect to its conflict of law provisions. You agree that any legal action or proceeding between you and Cigna will be brought exclusively in a federal or state court of competent jurisdiction sitting in Philadelphia, Pennsylvania, U.S.A. If any provision of this agreement shall be unlawful, void or for any reason unenforceable, then that provision shall be set apart from this agreement and shall not affect the validity and enforceability of all remaining provisions.

Release of Information Request
By accepting these terms and conditions, I authorize Cigna to release my personal health information to me via MyChart. I understand that this authorization is for the purpose of providing me with web-based access to my personal health information.
